Connecticut State Department of Health Ephemera - Health Tag for a Child Aged 4

Connecticut: 1930s. A health tag (2.25" by 4.25") produced by the Connecticut State Department of Health for one of its child health initiatives. On the recto is a printed form with empty fields for health information including health, age, height and weight, which has been filled out by a nurse, probably during a school visit; the name field has been filled out by the child (which is charming, but also sadly means we have been unable to decipher it). On the verso is a picture of a healthy child with a list of healthy habits, including bathing regularly and eating "green vegetables and dark bread" Very good. Some wear and dust soiling. Creased where previously folded. Cord a little worn.
